Abstract

|
본 연구는 지속적이며 일관성 있는 통합적인 입원환자 관리를 위해 표준화된 환자관리와 질 향상 및 비용 효과성을 높이기 위한 Critical Pathway를 개발하는 연구이다.
연구를 위한 이론적 개념틀로는 미국 매릴랜드주 존스홉킨스 병원의 Critical Pathway를 개발하여 적용하는 과정을 이용하였다.
1단계로 개발할 진단명/시술명을 골수이식술로 정하고, 2단계는 골수이식술 환자관리에 직접 참여하는 8명의 전문가 집단으로 개발팀을 구성하였으며, 3단계로 의무기록지를 통하여 환자들에게 수행된 전반적인 의료/간호활동의 내용을 분석하여 의료 및 간호영역별 행위의 내용과 시행기간을 결정하였다. 4 단계에서는 전문가 집단의 내용타당도 검증을 거쳐 예비 Critical Pathway를 작성하였고, 5단계에서 예비 Critical Pathway의 실무적용 가능성을 확인하기 위해 골수이식을 하기 위해 입원한 8명중 기준에서 탈락한 1명의 환자를 제외하고 7명의 환자를 대상으로 임상 타당도를 검증한 후 6단계에서 최종 Critical Pathway를 확정하였다.
구체적인 연구결과는 다음과 같다.
1. 의무 기록지 분석을 통하여 조사된 의료 및 간호의 내용을 감시/사정, 처치/간호, 투약, 활동/안전, 식이/영양, 검사, 협의진료, 교육의 8개 내용으로 영역화 하여 이를 Critical Pathway의 종축으로 하고 횡축은 입원에서 골수이식 후 7일까지로 정하였다.
2. 예비 Critical Pathway작성을 위한 의무 기록지 53례를 분석한 결과 평균 재원기간은 45일이었고 입원 일부터 골수 이식 후 7일까지는 평균 재원기간의 22.7일로 나타났으나 가장 빈도수가 높은 것이 21일로 나타나 입원 일부터 시술 후 7일까지 횡축의 총 재원기간을 21일로 정하였다. 예비 Critical Pathway적용은 예정된 골수이식술 환자에게 입원 일부터 적용하였다.
3. 8개 영역의 의료내용을 164 문항의 질문지로 작성하여 전문가 집단의 내용 타당도를 본 결과 88% 이상의 합의를 보인 151문항은 채택하였고, 75% 이하의 합의를 보인 13문항은 수정 또는 삭제하여 예비 Critical Pathway를 작성하였다.
4. 예비 Critical Pathway의 임상 타당도 검증은 1998년 1월 15일부터 1998년 8월 30일까지 8개월 동안 기준에 맞는 골수이식환자 8명을 대상으로 실시하였으나 1명은 연구기준에서 벗어나 탈락되어 실제 연구대상은 7명이었다. 이 중 2명의 재원기간은 예정보다 1-2일이 빨랐으며, 1명은 예정보다 1일 늦게, 다른 1명은 2일이 늦어져 이들의 평균 재원일수는 21일이었다. 실무적용시 차이를 보인 의료내용을 13문항 중 12문항은 수정·보완하였고, 적용이 불가능하다고 판단되는 1문항은 삭제하여 골수이식술 환자를 위한 최종 Critical Pathway를 개발하였다.

Bone marrow transplantation, a promising therapeutic option for marrow disorders, has till several problems such as complex and time-consuming schedule, high cost, need for aseptic management, and high incidence of complications. A systemic management of the process could reduce the duration of admission and complications, and enhance the quality of medical services as well as cost effectiveness. Therefore, the purpose of this study was to develop a critical pathway for patients undergoing bone marrow transplantation, which aims to standardize and systematize the patient care in bone marrow transplantation.
The first step of developing the critical pathway was to select the disease/treatment modality, which was bone marrow transplantation in this study. In the second step, a development team consisted of eight experts in bone marrow transplantation was organized. In the third step, the service content and the implementation period for the bone marrow transplantation was decided by reviewing medical records. In the forth step, a preliminary critical pathway was made through verification of expert group on content validity. In the fifth step, the clinical appropriateness of the preliminary critical pathway was validated by applying it to seven patients undergoing bone marrow transplantation. In the sixth step, the final critical pathway was developed.
Through the process the following results were obtained.
1. Eight categories of medical services, i.e. monitoring/assessment, treatment/care, medication, activity/safety, diet/nutrition, test, consult, education were set to vertical axis, and the hospital stay from hospital admission to the 7th day after bone marrow transplantation to the horizontal axis.
2. The mean hospital stay (from hospital admission to the 7th da after BMT) of 53 patients whose medical records were reviewed was 22.7 days, and the overall hospital stay (from hospital admission to discharge) was 45 days.
3. Among 164 questionnaires on medical service contents in 8 categories, 151 showed expert’s agreement over 88% and were selected to analyze. Thirteen questionnaires were modified because showed expert’s agreement below 75%.
4. A clinical validity test was done in eight patients. during the process, one patient was deleted because he was out of the investigator set criteria. Finally seven patients were verified using a preliminary critical pathway. The mean hospital stay of them was 21 days. Nineteen items showed differences between preliminary critical pathway and the real medical activities. Then of those nineteen items were modified of supplemented, six were deleted, and three were added to make the final critical pathway.
